Behavioral Health Consultant
Conduct brief interventions using self-management strategies with adults suffering from depression with comorbidity of uncontrolled diabetes, hypertension, and obesity. Deliver behavioral health services in 2 Primary Care Neighborhood Clinics and at Grady Hospital's Outpatient Internal Medicine Clinic. Engage in co-management consultations with Primary Care Physicians, Specialists, and Psychiatrists. Administer screenings, behavioral health, substance abuse, and suicide risk assessments (PHQ-9, M3Clinician, Healthify, SLAB and other assessment tools). Document in EPIC.
- Act as liaison between Primary Care and specialty mental health and substance abuse services - deliver triage, on-demand behavioral health consultations, prevention, adherence enhancement, and patient education.
- Practice a culturally-centered approach and engage in motivational interviewing, CBT, person-centered, solution-focused, and open-dialogue therapy (recent).
- Integrated behavioral health approach into 3 clinics; championed cultural shift within the health center to deliver more holistic care to patients. Build integrated behavioral health services throughout the organization.
- Clinical Provider on Project Thrive team with Morehouse School of Medicine; provide research and data analysis.
